Tennessee's top prisons official is stepping down after the state's failed attempt to execute Christa Pike, Gov. Bill Lee announced Saturday, according to The Associated Press.
Department of Correction Commissioner Frank Strada will resign this month. Pike, who survived two doses of the execution drug pentobarbital on Wednesday, remains unconscious on a ventilator in a hospital, according to court filings from her attorneys reported by AP and CBS News.
## Why Strada is leaving
Strada said stepping aside is "in the best interest of Tennesseans" while an independent review examines what happened, AP reported. He also maintained that the execution team "carried out its responsibilities in accordance with the established protocol."
Strada took over the department in January 2023 after working in corrections in Arizona, according to AP. In late 2024, he announced that Tennessee would switch to pentobarbital as its only lethal injection drug.
AP reported that no successor or exact departure date had been announced as of Saturday.
## What Pike's lawyers say went wrong
Pike's legal team says the IV lines used in the execution were not placed correctly, so the drug went into her body tissue instead of her bloodstream. In a court filing quoted by AP, the attorneys wrote that "at no point did any member of the execution team realize that the IV lines were not correctly placed."
According to CBS News, the IV team used at least seven needles, and Pike's arms were "swollen, burned and blistered" when she arrived at the hospital. PBS News reported that the needles went into her left arm over roughly an hour. Her lawyers say they had warned the state in advance about her difficult vein access, CBS reported.
State officials have not publicly explained what caused the failure, according to PBS News.
## The state's response so far
Lee has ordered an independent review of the execution and called off the remaining execution scheduled in Tennessee for 2026, according to CBS News and PBS News. Pike's own execution has been postponed indefinitely.
Her attorneys have asked a court to preserve all evidence related to the attempt and are seeking to have her death sentence commuted, calling the execution "cruel" and "torturous," CBS News reported.
## How Tennessee's backup plan failed
Tennessee's protocol includes two safeguards: two IV lines, and permission to give a second dose of pentobarbital if the prisoner is still alive five minutes after the first, according to PBS News. Both were used in Pike's case, and neither worked.
Tennessee has faced scrutiny before. A 2022 independent review found the state had not followed its own lethal injection protocols since 2018, PBS reported. Failed or prolonged executions have also happened elsewhere: Idaho halted an execution in 2024 after eight unsuccessful IV attempts, and a 2014 Arizona execution involved 15 injections over nearly two hours, according to PBS.
## Background
Pike was 18 when she and her boyfriend killed a classmate in 1994, according to AP. She became one of the youngest women ever sentenced to death in Tennessee and has spent nearly three decades on death row.
The victim's mother expressed frustration after the failed execution, saying, "I didn't get justice for my daughter," according to PBS News. Pike's lawyers, meanwhile, describe the attempt as "cruel" and "torturous" and argue her sentence should be commuted.
## What happens next
Strada's departure does not end the independent review, which will try to determine why the IV lines failed. Key open questions include who will lead the Department of Correction next, what the review finds, how Pike's medical condition develops and how courts respond to her lawyers' request to commute her sentence. NextGen America will keep following the Pike case as it develops.
